What is chronic care management?

Chronic care management is a program for people with two or more medical conditions that your provider expects to last at least 6 month, or conditions with a high risk of severe disease progression or bodily deterioration. 

The chronic care management program helps you better manage and live with your conditions through medication, lifestyle counseling, and other medical care. It may also prevent more time spent in the hospital and allow you to enjoy a better quality of life. 

What conditions qualify for chronic care management?

Chronic care management can cover many medical conditions, with some of the most common including: 

  • Hypertension (high blood pressure)
  • Thyroid disease (hypothyroidism, hyperthyroidism, and other types)
  • Type 1 diabetes

What does chronic care management include?

Chronic care management includes both in-office care and telehealth. At the beginning of the chronic care management program, you’ll come to the Quality Health Source office for a comprehensive exam. In that visit, Dr. Johnson performs a full medical exam and creates an effective disease management program for you to follow at home. 

You’ll learn how to monitor your health at home (for example, checking your blood sugar and blood pressure) and how to share that information with Dr. Johnson during telehealth visits (by phone or computer conferencing). In addition, Dr. Johnson teaches you about healthy changes that can improve your symptoms and quality of life. 

If problems develop, Dr. Johnson can bring you into the office or coordinate any specialist care you might need.
